When calling for an appointment please have both your dental and medical insurance information available. This includes: the subscribers name, date of birth, social security number (ID and Group Number), place of employment, address to submit the insurance claim to, and provider phone number so we can determine your benefits.
Please bring all of your insurance information with you on the day of your appointment so that we can expedite the processing of your claim.
For your convenience we accept Visa, MasterCard, check or cash. Our fees are reasonable and competitive. With this in mind payment is due at the time services are rendered unless arrangements have been made in advance. For any questions regarding your account please contact our office at 440-234-1810.
Any patient that is 18 years of age is considered the guarantor of his or her account, unless, legal documentation proves otherwise. In divorce or separation situations both parents are asked to discuss financial arrangements prior to the appointment. The parent bringing the child in for services will be considered the guarantor.
Remember payment is due on the day of surgery unless other definite arrangements have been made. No accounts are carried after 90 days.
We'll be happy to assist you in filing your insurance claim, however, it is your responsibility to provide our office with your Insurance information at the time of your appointment. Any difference between your fee and the amount your Insurance pays is your responsibility.
Please keep in mind that your Insurance contract is between you and your Insurance Company, not the Doctor. It is your responsibility to be familiar with and understand the terms, conditions, and limitations of your contract.
